1. The system is available at https://zezwolenia.pzw.suwalki.pl it offers purchasing E-permissions for angling.
2. One-day and three-day permissions are available for non-members as well as fees for anglers associated in Polish Angling Association.
3. To purchase the permission you must confirm having accustomed with the rules and regulations available at Suwałki Unit's web site. :

4. Purchased and printed permission is valid with payment confirmation and angler's licence.
5. E-permission can be purchased by means of online payment.
   1. online payment enables quick and safe payment for the service ordered.
   2. no additional costs are carried when paying for the E-Permission using a credit card or an online transfer,
   3. when paying for the E-Permission the buyer should get accustomed with times of realisation of payment.
6. All payments are realised by PayU (former Platnosci.pl)
7. PayU system offers three types of payment :
   1. module of payment made by means of electronic non-cash payment in Polish currency using internet transfers.
   2. module of traditional payment outside the Internet or other electronic transfer.
   3. module of payment made by means of cash cards (Protected Payment) including non-cash payment in Polish currency made with cash cards admitted by banks to realisation of transactions in the Internet environment.
8. After confirming the transfer by PayU system E-Permission will be available at the website immediately after payment or in due time e.g. when paying by means of traditional payment.
   When payment is not confirmed its status can be checked at the website where the client will be redirected after proper payment by [Check Payment] button.
   If payment is confirmed the the E-Permission can be printed using the button included in the website. At the same time a message with the address where an image of the E-Permission for angling - which must be printed is sent to the email address inlcuded in the aplication form.
9. Personal data included in the application form enable identification and confirmation of legal status of the purchased right to fish in the form of E-Permission. All personal data included in the application form shall be included in the printed version of E-permission. User's personal data can be changed after personal contact with the office of PAA Suwałki Unit
10. All personal data of the permission holder can be changed in premises of PAA Suwałki Unit
11. All information conncted with the purchase of the E-Permission are available at the office of PAA Suwałki Unit, ul. M.Konopnickiej 10, 16-400 Suwałki, lub pod numerem telefonu : +48 87 5664005.

I hereby give consent for my personal data included in my application for the purchse of E-Permission to be processed by PAA Suwałki Unit (later referred as PAA) locaed at ul. M.Konopnickiej 10, 16-400 Suwałki under the Personal Data Protection Act as of 29 August 1997, consolidated text: Journal of Laws 2002, item 1182 as amended.

I acknowledge that I have the right to access and rectify abovementioned personal data, as defined in the Act of August 29, 1997 on the Protection of Personal Data (Journal of Laws of 2014, item 1182, with amendments). Submitting personal data is voluntary, but necessary to accomplish the purchase.

If you are foreigner, non-associated in Polish Angling Associaton (PAA), don't choose this option. In that case choose "Non-associated" option.

4. Pani/Pana dane osobowe przetwarzane są przetwarzana na podstawie Art. 6 ust. 1 lit. b ogólnego rozporządzenia o ochronie danych osobowych z dnia 27 kwietnia 2016 r. w celu realizacji zadań statutowych Polskiego Związku Wędkarskiego
5. Pana/Pani dane osobowe będą przechowywane w oparciu o uzasadniony interes realizowany przez administratora, jednak nie dłużej niż 6 lat *
6. posiada Pani/Pan prawo do: żądania od administratora dostępu do danych osobowych, prawo do ich sprostowania, usunięcia lub ograniczenia przetwarzania, prawo do wniesienia sprzeciwu wobec przetwarzania, prawo do przenoszenia danych, prawo do cofnięcia zgody w dowolnym momencie
7. ma Pan/Pani prawo wniesienia skargi do organu nadzorczego: Urzędu Ochrony Danych Osobowych ul. Stawki 2, 00-193 Warszawa
